My LD/ADHD son is transitioning from private to public school for sixth grade in July. Can I obtain a copy of the Section 504 form and present it to the school, or do I have to wait for the school to complete the form and present it to me? Since I already know he will need a modified curriculum based on his present classroom accommodations, I don't want to "wait" any longer than I have to.

I don't see why you can't bring a copy of the 504 form with your suggestions written on it. Certainly the 504 Plan should be a collaborative effort, but there's nothing wrong with you getting the process started on your own.
